Senior Falls Reduction and Prevention Program

Eligibility

This program is designed for Oconto County residents age 55 and older that are having mobility issues or are falling frequently. There are no insurance or income eligibility limits.

What is Included

  • An in home assessment done by Public Health to determine needs and link to community resources.
  • Public Health will assist with obtaining and installation of fall prevention equipment in the home.
  • Public Health will follow up and monitor progress, answer questions, and provide ongoing resources to prevent falls.
